We are now using a new online consultation tool. It is NHS approved and easy to use.
You can contact us about a medical, administrative or prescription issue by clicking below and filling out a short form.
When submitting the form, you will need to answer a few short questions and we will get back to you within two working days. Please do not use this form for urgent or emergency requests, contact 111 or 999 if it is a medical emergency. The forms that are submitted will only be read during office hours, the forms will not be read on Saturdays and Sundays, or bank holidays.
What do I need to be able to use this video consultation tool?
- A smartphone, either with working Wifi or 3G/4G connection (if you do not have a smart phone, your GP may be able to email the link to you for you to use on a tablet or laptop)
- Your Internet Browser will need to have your phone's microphone and camera enabled (to see how to change this in your phone settings please visit the Accurx website)
- Please note, iPhone's that have iOS 12 software or older, will need to download the Whereby app to join the consultation
